Ongoing My Symptom Check-In Instructions

When to use this

Use these instructions during the 6-weekly check-in with a patient, either:

  • In practice, with the patient present
  • Over the phone, if the practice staff is completing the check-in with the patient at home when the patient has been called directly by nurse
  • Retrospectively – i.e. patient completed paper version

Choose the right assessment pathway

Option 1: Patient completes on their own phone (recommended)

Use this when the patient is comfortable completing the assessment themselves when visiting the practice 6 weekly. Same process as previously discussed.

Steps

  1. With patient record open in BP, Open GoSharePSI and log in.
  2. Switch to ‘Use current patient in BP’ mode
  3. Select ‘My Symptom Check-in – Assessment 2 (Usual provider inbox)’, tick SMS, then click Send Bundle.
  4. Ask the patient to complete Assessment 2 on their own phone.
  5. The patient will be shown relevant education for any symptoms rated moderate or severe.

Note:

  • If the patient rates any symptom as severe, automated follow-up will be triggered. The patient will receive:
    • a text message at 10:00am the next day asking them to rate symptoms again
    • a voice AI call at 1:00pm if they do not complete the follow-up from the text link

Option 2: Nurse completes on the patient’s behalf

Use this when the patient is not comfortable using their own phone.

Steps

  1. With GoSharePSI open and the patient record loaded in BP, Select the bundle: Online Only My Symptom Check-In Assessment 2 (24hr follow up)
  2. Tick ‘Send to alternate recipient?’ check box at the bottom of the screen
  3. Enter your email address and/or mobile ie. nurse@gppractice.com.au, click save, then Send bundle. ** You do not need to enter both a mobile and email in this screen.
  4. Find the email or SMS from ‘GoShare Healthcare’ in your email inbox, click link in email to open Assessment 2. Complete on patient’s behalf – Completed assessment will still be auto imported into the BP inbox of the patient’s usual provider.

This option can be used

  • during the 6-weekly in-practice consult
  • during a phone call when the patient is at home

Important

  • The Online Only bundle does not include:
    • automated voice call 3 hrs after send if not completed.
  • If patient responds with ‘severe’ for any symptoms, they will receive an SMS the following day and a voice call follow up, 3 hours later.

Quick decision guide

  • If the patient can complete the assessment themselves → use My Symptom Check-In Assessment
  • If the nurse needs to complete it for the patient → use Online Only My Symptom Check-In Assessment 2 (24hr follow up)

Key difference to remember

The main distinction between the two bundles is 3hr later voice call:

  • Standard bundle = If online version not complete, automated follow-up 3hrs after initial bundle send
  • Online Only bundle = No automated follow-up voice call after initial send.
